Effective immediately I officially inform any connected agents or agencies not affiliated with Dain Martindale, Martindale Insurance Services LLC dba Martindale Financial Group that I no longer grant permission, in any manner, for anyone other than Dain Martindale, Martindale Insurance Services LLC dba Martindale Financial Group to access, search for, or make changes to my Marketplace plan.

This includes any previously given consent to any other agents or agencies in the past.

I grant only Dain Martindale, Martindale Insurance Services LLC dba Martindale Financial Group the right to manage my health insurance and marketplace application exclusively. I understand that my consent remains valid until I revoke it, and I reserve the right to revoke or modify my consent at any time.
